"Improving the implementation of transitional care innovations"

Transitional care innovations (TCIs) are emerging innovative solutions designed to enhance care continuity and coordination for older persons with chronic medical conditions when moving between multiple healthcare settings and providers such as the hospital, homecare, or nursing facilities. However, implementing these innovations into practice settings and making them work in the real world is challenging and often subject to failure. This dissertation explored which factors influence the implementation of TCIs and how to improve it. The findings indicated that organizational leadership and its overall readiness and capacity to implement innovations are key factors. In addition, the continuous engagement of key stakeholders and the presence of highly committed and motivated individuals with specific facilitating roles can drive forward the implementation. Furthermore, this dissertation concluded in developing a set of forty strategies to improve the implementation of TCIs in the future, and ultimately improve care delivery for older persons.
