"Medication Optimization; exploring the use of a clinical decision support system"

The aim of this PhD dissertation was to improve the safety of medication use in hospitals as well as nursing homes by using a digital system to support clinical decisions.

Automatizing medication reviews (periodic check-ups of all medicines used by a patient) was shown to save a huge amount of time. Next, the conditions for an efficient supporting software system were studied, and software meeting these conditions was actually developed. Although the software was shown to support manually performed medication assessments, further optimisation is required. Furthermore, this study developed a formula allowing the automatic identification of patients at high risk of developing delirium. In the future, this may also provide a solution for other relevant clinical problems.
