Please note:

Your request will be reviewed within 4 weeks
If required, upload documents to complete your request. In case of:
- (prolonged) Illness or injury: a medical certificate;
- Deregistration at UM: a proof of deregistration;
- Relocation (>20 km from UM SPORTS): proof of registration at your new municipality.
